Eligibility Criteria:

1. The age of the patient must be 60 years or above.
2. The patient must possess a valid ration card.
3. He/ she must have an Aadhaar card and voter ID card.
4. The form has to be downloaded, filled up and has to be signed by the District Social Welfare Officer of the district which is mentioned in the address proof of the applicant.
5. The concerned District Social Welfare Officer will sign the form after all necessary address proof, identity proof, ration card, Doctor’s certificate certifying that the applicant requires cataract surgery is submitted by the applicant.
6. The applicant will then submit the signed application to the nearest allotted health institution for availing the benefit of the scheme.
